Caloric restriction, exercise help prevent weight gain, other complications in obese women

Feb. 11, 2013 ? In a study to be presented on February 15 between 1:15 p.m., and 3:30 p.m. PST, at the Society for Maternal-Fetal Medicine's annual meeting, The Pregnancy Meeting ?, in San Francisco, researchers will report findings that suggest that consistent physical activity and healthier lifestyle changes beginning in a woman's first trimester can prevent excessive weight gain in obese pregnant women (with a Body Mass Index greater than 30), helping to avoid preterm delivery, hypertension and gestational diabetes.

This randomized study, entitled Occurrence of pregnancy complications in women with BMI >25 submitted to a healthy lifestyle and eating habits program, should help physicians understand why it is important to offer more specific advice to obese women who are pregnant regarding the amount of weight they should gain during their pregnancy.

"Weight gain should be monitored related closely to the starting weight. If you start with normal BMI [defined by weight before pregnancy divided by height measured in squared meters] could increase 11-16 kilograms. But if you are obese, the expectation is that you should only gain 5-9 kilos -- more or less just the weight of the pregnancy" said Dr. Fabio Facchinetti, referencing the Institute of Medicine (IOM) in the United States.

Researchers from the University of Modena and Reggio Emilia's mother-infant department teamed up with their colleagues in the clinical diagnostic medicine and public health department to create a Therapeutic Lifestyle Changes (TLC) Program. The 33 women in the treatment group followed a regimen of 1500 kcal/day diet coupled with mild physical activity (30 minutes/day, 3 times/week). Twenty-eight women in the control group just received general advice from their OB/GYN. They only worked with single pregnancies and treatment began at 12 weeks.

Their research shows that of the obese subgroup of women [those in TLC group with BMI>30] who stuck with a proper diet and exercise, 77.8 percent were able to stay within the allotted weight gain limit, as opposed to only 30 percent from the control group. Additionally, the rate of, hypertension, preterm delivery and gestational diabetes was significantly lower in the TLC group.

"The diagnosis of gestational diabetes is done [by having the women drink] 75 grams of glucose and looking at the levels after one and two hours," said Facchinetti. "In our study, we found [after monitoring the glucose levels] that 57 percent of obese women in the non-treatment group had diabetes whereas in the treatment group, it was only 21 percent. This is important because [the glucose test] was done at 16-18 weeks and, even if negative, repeated at 24 weeks. In other words, if the first evaluation occurred just 12 weeks after the treatment began, the small amount of time was enough to correct the body's level of glucose."

The study was conducted by Fabio Facchinetti, Elisabetta Petrella, Lucrezia Pignatti and Isabella Neri of the University of Modena and Reggio Emilia, Mother-Infant Department, Modena, Italy; and Marcella Malavolti, Valentina Bertarini and Nino Carlo Battistini of the University of Modena and Reggio Emilia, Clinical Diagnostic Medicine and Public Health, Modena, Italy.

CHICAGO (AP) ? Two Chicago gang members charged Monday in the death of a 15-year-old honor student mistook her and her friends for members of a rival gang and attacked the group in retaliation for a shooting that injured one of the men over the summer, according to police.

Hadiya Pendleton died after being shot in a park about a mile from the Chicago home of President Barack Obama on Jan. 29, just days after she performed during his inauguration festivities in Washington. Her death was among dozens of homicides in Chicago last month, though her background and ties to Obama thrust her death into the national headlines.

Michael Ward, 18, and Kenneth Williams, 20, were taken into custody late Saturday, while on their way to a strip club ? and just hours after first lady Michelle Obama and other dignitaries attended Pendleton's funeral. Both men are now charged with first-degree murder, two counts of attempted murder and aggravated battery with a firearm, Chicago Police Superintendent Garry McCarthy said Monday night.

"Ward confessed and indicated Hadiya was not the intended target. They got it all wrong," McCarthy said.

Pendleton, a popular high school majorette, was with a group of friends who took cover during a rainstorm under a canopy in a park about a mile from the Obama home on the city's South Side. Police said a man hopped a fence, ran toward them and opened fire with a handgun before fleeing in a waiting car. Pendleton was struck in the back and died later that day. Two others were injured.

McCarthy said Ward told investigators he thought he was shooting into the crowd of a rival gang, and that the shooting was meant as retaliation for Williams being shot in the arm in July. Police said neither Pendleton nor her friends were affiliated with gangs.

Williams, who refused to cooperate with authorities after the July shooting, was driving the getaway car, McCarthy said. He added that both men were arrested while on their way to a strip club to celebrate a friend's birthday Saturday night.

Pendleton's death was one of more than 40 homicides in Chicago in January, a total that made it the deadliest January in the city in more than a decade. But her murder attracted national attention and helped put Chicago at the center of a national debate over gun control.

Not only did the first lady attend the teen's funeral, but the girl's parents were set to sit with Michelle Obama during the president's State of the Union address on Tuesday night. Obama is scheduled to return to Chicago three days later to discuss gun violence.

Homicides in Chicago topped 500 last year for the first time since 2008, stoking residents' concerns about gun violence and leading the police department to put more officers on the street and to focus more on combatting gangs.

McCarthy, who is pushing for tougher gun laws that would increase minimum sentences for gun crimes, noted that Ward was arrested in January 2011 on a gun charge but he received probation after pleading guilty to unlawful use of a weapon. If Chicago had laws like those in New York City, McCarthy said, Ward wouldn't have been on the streets.

"This has to stop. Gun offenders have to do significant jail time," said McCarthy, who rose through the ranks of New York City's police and is the former police director in Newark, N.J.

McCarthy said the arrests occurred after police figured out that the description of the car in which the shooter fled matched the description of a vehicle in which Williams had been pulled over a day before the shootings. The police superintendent noted it didn't come from a tip from the community.

"I'm sad to point out we did not get our target audience to step up," he said.

Just as the December killing of 20 children and six adults at an elementary school in Newtown, Conn., brought renewed scrutiny of the nation's gun laws, the death of the popular Chicago teen has cast Chicago's gun violence problem in a new light.

Earlier Monday, Chicago Mayor Rahm Emanuel seemed to make just that point.

"The only time when the gun issue ever gets affected is when Newtown happens," he said. "What happens in urban areas around the country too often ... gets put to the side."

He said that while it's not wrong that massacres stir such debate, what happens on the streets of Chicago and in other urban areas "gets put in a different value system."

"These are our kids," he said, his voice rising. "These are our children."

Emanuel joined McCarthy and Cook County State's Attorney Anita Alvarez at an afternoon news conference to announce they would push for tougher gun laws that would increase the minimum sentences and require offenders to serve at least 85 percent of their sentences.

They say the law now allows offenders to be released after serving no more than half their sentences and sometimes obtain their release after a matter of weeks.

Software?tracks terrorists ? and you

Web software designed by the defense firm Raytheon can scour Facebook, Twitter and check-in sites such as Foursquare to create an instant dossier covering a suspect's likely geographic locations and activities.

Raytheon calls it Rapid Information Overlay Technology (RIOT), and privacy advocates think it goes too far. "This sort of software allows the government to surveil everyone," Ginger McCall, attorney and director of the Electronic Privacy Information Center's Open Government program, told NBC News Monday.

"It scoops up a bunch of information about totally innocent people. There seems to be no legitimate reason to get this, other than that they can."

While some "terrorists," or those who might be threats to national security, use social networking, it's doubtful many of them show check-ins at local clubs or coffee shops, said McCall.

"The likelihood of this actually pulling in actual terrorists is low ? it seems to be a lot of white noise," she said.

NBC News contacted Raytheon about RIOT. In an emailed statement Monday, the company would only say this:

A report about RIOT was first published Sunday by The Guardian, which shared a video it obtained about how RIOT works. (We can't embed the video, but you should have a look at it on The Guardian's site.)

In the video, Raytheon's Brian Urch, principal investigator for RIOT, shows how easily the software can track one of Raytheon's employees, named Nick. The system combines Nick's Foursquare check-ins with the GPS data encoded automatically on his posted photos. The system creates a pie chart of Nick's most favorite spots to visit, and reveals without question what time he likes to hit the gym.

"6 a.m. appears to be the most frequently visited time at the gym, so if you ever did want to try to get a hold of Nick ? or get a hold of his laptop ? you might want to visit the gym at 6 a.m. Monday," Urch says ominously.

A Raytheon spokesman said in an email to the Guardian that RIOT is not being used right now, but rather is "a big data analytics system design we are working on with industry, national labs and commercial partners to help turn massive amounts of data into useable information to help meet our nation's rapidly changing security needs."

And just because this software neatly packages information in ways that can prove awkward or even incriminating, it is not breaking through privacy barriers to get it. Instead, it uses what people have voluntarily posted for public consumption. The key is, therefore, knowing what you're posting.

Users need to be careful, said privacy lawyer McCall. Social networking sites "work very, very hard to push more data into the public realm, and often users have no idea how much data is being made publicly available, and the companies make it very difficult for users to control their own information."

For example, she said, "other people can check you in" at a location, even though "it may not be data that you put out there. On Facebook, it allows your friends to check you in, and if your friends don't set their privacy setting" so that others can't see that, "you can be tagged and checked in."

"I went to a concert with a friend a month ago, and she likes to publicize those things on Facebook," McCall said. "So it was on there that she was at a certain club with Ginger McCall."

"When (most users) sign up for sites like Facebook or Foursquare," she said, "they never would guess that their information is about to be aggregated," and perhaps even "used to track them down" with software such as Raytheon's RIOT.

NEW YORK (AP) ? Cellphone companies say their networks are largely up and running after a blizzard dumped up to 3 feet of snow on New England.

Not all cell towers have backup power, so natural disasters can take out cell service through power outages. Last fall's Superstorm Sandy had a major impact on cell service in flooded areas of the Northeast.

This week's snowstorm had much less effect. AT&T Inc. says the "vast majority" of its cell towers in the hardest-hit states are operating fine, and work is under way to restore service in some areas. Verizon Wireless, which prides itself on providing backup power to nearly all sites, says its network is performing well. Sprint Nextel Corp. says the storm's impact was "minimal."

This wine has citrus flavors and is lively and crisp but not as assertive as many New World Sauvignon Blancs. It would be perfect with trout or Dover sole saut?ed with butter and lemon.

2011 Chateau Soucherie Anjou Blanc, $19.99: Dry Chenin Blanc can be a remarkable creature. This has a lovely floral nose with crisp flavors. It has more body than a Sauvignon Blanc and more food options. This comes from near France?s Loire River. It would shine with a well made boudin blanc (carp sausage). And they offer some of the tastiest along the Loire River. Not like the gnarly carp we used to haul out of Lake Michigan as children. What horror.

2010 Chateau Soucherie Anjou Rouge, $19.99: This is the red analogue of the above white. It tastes much like a neighboring Chinon. We are assuming Cabernet Franc is the grape. So it?s a mid-sized red with a hint of graphite in the nose. Like the white above it is a great food wine. We would like it with some becasse (woodcock).

2011 Jean-Marc Burgaud Morgon, Les Charmes, $16.99: In all essences, a personality wine. While Morgon Beaujolais can be notably grapey, this offers a panorama of flavor essences. There is soil and sun and maybe a couple flat tires from bicyclists riding through the lovely hills of Beaujolais.

2008 Domaine La Boutiniere Chateauneuf-Du-Pape $34.99/Sale $29.99: A gentle Chateauneuf, reflecting the vintage of 2008. Red fruit delight with some herb and spice. We?d love to down it with some lamb chops and mint during a Valentine?s feast or Presidents? Day lunch, of course.

2011 Pesquie Blanc, Igp Mediterranee, $13.99/Sale $11.99: The white sibling of our beloved Pesquie red that we?ve been selling for centuries. It is 70 percent Viognier with some Rousanne and Clairette in the mix. It tastes like a high class Viognier with white flowers and white fruits in the nose. Dry, refined flavors. None of the bubble gum and bananas that you battle with Viogniers from hot climes around the globe. It?s another hit for Pesquie. The Chaudiere family, who owns Pesquie, certainly seems to know what wine is all about.

2010 Alain Voge Cornas, Les Chailles, $67.99/Sale $55.99: Cornas is the French emperor of Syrah, because it is 100% Syrah. It?s grown across the river from Hermitage. This is an almost black, rich, potent wine. It?s smoky, gamey with some charcoal notes. Yet there are blackcurrants, raspberries and violets in the nose that moderate the force. Not for the timid, but not abusive on the palate. It?s a delight with red meats, stews and game. It?s one of the world?s less known great wines.

2010 Stemmari Nero D?Avola, Sicily, $10.99: Nero d?Avola is a very big grape in the island of Sicily. And it should be. It?s a soft, but mouth-filling red to match the hearty and fine Sicilian cuisine. If you are stumped what to do on a special pasta night, this is your bambino.

Here they give a dark, rich and mouth-filling Dolcetto with earthy overtones. Some have called Dolcetto ?the Piedmont?s Merlot.? Well, it?s a little different from Merlot, but is softer than the region?s Barolos and Barbarescos.

2010 Quattro Mani Barbera D?Alba, $13.99/Sale $10.99: Our love for Italy?s Barbera wines is virtually eternal. Mainly because they are so good with food. That is, they have the acidity to cleave hearty flavors, but don?t become the main event. This dandy devil falls right in there. It bugles red fruit notes and has substantial fruit to go with the acid. Bring on a heaping plate of tajarin.

2009 Polkura Syrah, Colchagua Valley, $21.99/ Sale $19.99: This Chileno Syrah could stand right in there with some of the best Shiraz from Australia. It?s big with the expected smoke and spice. That?s easy. The class and finesse of this is what impresses. Muy Sabrosa.

2011 Whitehaven Sauvignon Blanc, Marlborough, $19.99/Sale $16.99: Whitehaven makes a Sauvignon Blanc of style which is still true to its provenance?so, there are some floral notes, minerality and a touch of that ?Kiwi? grapefruit. Not as assertive as many New Zealand Sauv Blancs.

2011 Jules Taylor Pinot Noir, Marlborough, $19.99/ Sale $17.99: Often we believe New Zealand Pinot Noirs don?t get the respect they deserve. Yes, at times they are too expensive for their quality level, particularly those from Central Otago. But this Marlborough Pinot hits the price/quality sweet spot. It?s not just a fruity, vivacious young thing, but has some substance with darker fruit and depth of flavor. Darn good Pinot Noir.

Because they give power and elegance. This red shows the exuberance of youth, but is tempered a tad by four months in new French oak barrels. This is a great value!

2009 Valle Do Bomfim, Douro, $13.99/Sale $11.99: Despite a dead economy, remarkable things are happening in Portugal?s world of wine. It?s kind of an adaptation to reality. This lovely, mid-bodied red comes from the Port zone, where Port sales are not what they once were. So, why not make dry red? Here we get a delicious red for dinner, without a hefty price.

But this wine has some spine and fundament, along with the ripe Malbec fruit and colors that everyone seems gaga about. We also have their cr?me de la cr?me, a Bordeaux blend called Amano.

2005 Quinta De Noval, Late Bottled Vintage Port, $21.99: From the first Port house to make LBV, comes a lovely, complex wine which is, in part, due to the fact that it is unfiltered. Four and a half years in cask makes it more integrated ?it is delicious, both richer and just slightly drier than most LBVs.

Camping gear found in ex-cop's truck

BIG BEAR LAKE, Calif. (AP) ? Authorities say camping gear was found along with weapons inside the burned-out truck belonging to Christopher Dorner, the former Los Angeles police officer suspected in three killings who is the subject of a manhunt in Southern California's snow-covered mountains.

Los Angeles police Sgt. Rudy Lopez said Sunday that the truck was so charred that investigators couldn't be more specific about the nature of its contents.

SWAT teams with air support and bloodhounds fanned out for the fourth day to search for Dorner, who has vowed revenge against several former LAPD colleagues whom he blames for ending his career.

Authorities planned a 1 p.m. news conference to announce a reward for information leading to the arrest of 33-year-old fugitive.

Chief Charlie Beck has ordered a review of the 2007 disciplinary case that led to Dorner's dismissal.
